The MDS COORDINATOR oversees the RAI process assuring schedules are followed, timeliness in completion of the MDS, CAAs and care plans developed in accordance with federal and state guidelines. This is a fulltime position that reports to the administrator of the facility.
ESSENTIAL FUNCTIONS and SKILLS
Communication skills to work with nursing staff for implementation of care plan
Oversees care plan meeting with resident and family
Assures that care plans are keep current and reflective
Verifies that assessments transmitted to database with necessary corrections
Education of staff related to ADLs, other areas of MDS as needed
Reviews of the MDS for accuracy for any trends that need education
Monitors documentation of Medicare and managed care resident and educate as needed
Monitoring of QM with emphasis on those impacting five-star rating
Experience in CMI for best outcomes and guidelines for documentation required
Knowledge of restorative programs and implementation
Participation in Quality Improvement program in the facility
Provide leadership in facility for financial implications of RUGS, PPS, case mix rates.
Have 2-4 years work experience in MDS and long term care
Point Click Care experience preferred
Willingness to attend seminars to remain current with regulations
EDUCATION
RN degree from an accredited college (minimum ASN)
Currently registered in the state
Have AANAC certification
Must have current CPR certification
