Summary
The VISN 10 Clinical Resource Hub (CRH) Behavioral Health Interdisciplinary Program Team (BHIP) Psychiatry position is a VISN based position located in VISN 10. This position falls under the general supervision of the VISN CRH MH Section Chief. A Clinical Resource Hub (CRH) is a repository of clinical and administrative staff that serve Veterans Affairs facilities within a Veterans Integrated System Network (VISN).
Responsibilities
Staff within the CRH are expected to provide interprofessional services to VISN facilities that are underserved or experiencing gaps in clinicians due to the inability to match provider supply with demand. Services provided by CRH staff are conducted via multiple modalities, including virtual and in-person care. Clinical Video Telehealth (CVT) is the primary modality for treatment, though this position may require travel to VISN medical centers or Community Based Outreach Clinics (CBOC).
The CRH Mobile Deployment Team (MDT) provides care through virtual, in-person, and Mobile Medical Units and are a Mission Act 402 requirement. In person deployments of clinical staff for short-term assignments will be at the discretion of the VISN CRH MH Section Chief and will depend upon clinical need and available resources.
The Clinical Resource Hub (CRH) Psychiatrist functions as a physician member of an interdisciplinary team consisting of therapists, clinical pharmacy specialists, case/care managers and/or addiction therapists most often as a member of the spoke site or Hub BHIP team. The CRH Psychiatrist may assume the role of primary provider, team provider or consultant depending on the Hub care model. The incumbent may conduct psychiatric evaluations, develop treatment plans, implement treatment, and provide clinical management for a variety of complex mental health conditions, including, but not limited to: Post-Traumatic Stress Disorder (PTSD), substance abuse, depression, anxiety, bipolar disorder, psychosis, and other related conditions. Treatment planning focuses on utilization of pharmacological, psychotherapeutic and technological interventions as well as appropriate referral for additional studies and/or for adjunctive psychosocial interventions. Treatment planning also involves informing patients of risks, benefits, and alternative treatments and answering all questions. Incumbent will provide consultation to Veterans' families/caregivers as well as mid-level providers and other professional staff on the assessment, education, and treatment of Veterans with complex clinical problems as appropriate. Position requires a working knowledge of advanced psychopharmacology, substance abuse treatment, psychotherapy and telehealth technologies.
FUNCTIONS OR SCOPE OF ASSIGNED DUTIES
- Perform duties as a practitioner and consultant in Psychiatry within a variety of clinical settings. May provide inpatient services, partial hospitalization services, intensive outpatient services, routine outpatient services, consultation/liaison and/or to disaster response locations.
- Responsible for performing the full range of consultation, evaluation and treatment of emergency, complex and routine cases in the field of Psychiatry for a variety of emotional, psychological, and behavioral disorders.
- Prepared to provide various treatment therapeutics including medication management, individual, marital and group psychotherapies as indicated.
- Provide therapeutic interventions beyond the individual Veteran to include family psychotherapy and caregiver stress support as indicated and appropriate.
- Exercise initiative and sound judgment in establishing evaluation techniques, developing treatment and rehabilitation plans and providing appropriate therapeutic treatment services.
- Provide participation, professional consultation and collaborative medical support to other professional staff, interdisciplinary teams such as psychologists, advanced practice nurses, social workers and non-behavioral health medical providers to maximize the effectiveness of their interventions and to increase health-promoting behaviors, with a significant emphasis on a prevention-oriented model of care. Such interdisciplinary teams may include Behavioral Health Interdisciplinary Program (BHIP) and Patient Aligned Care Team (PACT).
- Align and collaborate Veteran care as authorized with community providers/partners and programs.
- Support other programs and initiatives, e.g. Behavioral Health Laboratory, Care Facilitation consultation, as indicated.
- Remain abreast of and engaged in all clinical Mental Health policies, procedures and roll outs at the National, VISN and Local level, e.g. suicide prevention initiatives.
- Provide education regarding connected care services including web resources and mobile applications to patients and medical staff.
- Attends and actively participates in mental health activities (i.e., staff development, residency conferences).
- Provides formal education and clinical education in his/her area of expertise to medical students and psychiatric residents in training.
- Provides patient education about diagnoses, therapeutic options, medications and related topics when appropriate.

Qualifications
The individual must qualify to perform duties as a physician, have completed an accredited residency and/or possess experience which has qualified the individual to perform general duties and some specialized functions and procedures without supervision. Staff physicians may also include attendings who train physician residents assigned in facilities with residency training programs, and consultants who are capable of giving authoritative views and opinions on subjects in their field of medicine.
Basic Requirements:
- United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
- Degree of doctor of medicine or an equivalent degree resulting from a course of education in medicine or osteopathic medicine. The degree must have been obtained from one of the schools approved by the Department of Veterans Affairs for the year in which the course of study was completed. Approved schools are:
(1) Schools of medicine accredited by the Liaison Committee on Medical Education (LCME) for the year in which the degree was granted, or
(2) Schools of osteopathic medicine approved by the Commission on Osteopathic College Accreditation (COCA) of the American Osteopathic Association for the year in which the degree was granted.
(3) For foreign medical graduates not covered in (1) or (2) above, facility officials must verify with the Educational Commission for Foreign Medical Graduates (ECFMG) that the applicant has met requirements for certification, and must obtain a copy of the ECFMG certificate, if claimed by the applicant. - Current, full and unrestricted license to practice medicine or surgery in a State, Territory, or Commonwealth of the United States, or in the District of Columbia. The physician must maintain current registration in the State of licensure if this is a requirement for continuing active, current licensure.
- Completion of residency training, or its equivalent, approved by the Secretary of Veterans Affairs in an accredited core specialty training program leading to eligibility for board certification. VA Physicians involved in academic training programs may be required to be board certified for faculty status. Approved residencies are:
(1) Those approved by the accrediting bodies for graduate medical education, the Accreditation Council for Graduate Medical Education (ACGME) or American Osteopathic Association (AOA), in the list published for the year the residency was completed, or
(2) Other residencies or their equivalents which the local Professional Standards Board determines to have provided an applicant with appropriate professional training. - Proficiency in spoken and written English.

Physical Requirements: In Accordance With VA Directive and Handbook 5019, this position requires visual acuity, keen hearing, clear distinctive speech, and manual dexterity. This position requires mostly periods of continued walking, standing, stooping, sitting, bending, pulling, and pushing. Transferring patients and objects may be required. The incumbent may be exposed to infected patients and contaminated materials and may be required to don protective clothing in isolation situations. The incumbent may occasionally be exposed to patients/residents who are combative secondary to delirium, dementia, or psychiatric disorders. The incumbent must be a mature, flexible, sensible individual capable of working effectively in stressful situations, able to shift priorities based on stakeholder needs.
