The role of the clinical case manager is a critical role in our outreach program at Pathways to Housing DC. As a clinical case manager you will develop relationships with individuals experiencing homelessness in Washington DC in order to lead clients to successful transition into and maintain stable housing. Clinical case managers will develop individualized plans for each client to assess their needs as it pertains to housing, mental health, employment, and other supportive services. A strong candidate will implement evidence-based clinical practices and approaches to develop strong rapport with individuals experiencing homelessness, motivating clients into readiness for services and successful goal completion.

Day-to-day tasks include conducting daily wellness checks, providing clients with necessities to survive, goal setting, resource referrals, follow-up and monitoring, crisis intervention, obtaining vital documents, coordinating with outside agencies, and other case management services as needed. Additional responsibilities entail promptly addressing concerns raised by police, fellow social service providers, business owners, employees, residents, and downtown visitors regarding individuals experiencing homelessness in the vicinity at the direction of the Program Manager utilizing emergency dispatch protocols and procedures.

Approximately 75% of the time will be spent in the community, directly providing services to persons living on the street; the remaining time is allocated to fulfilling administrative duties, such as documentation, case conferencing, care coordination and case management.

Shift schedules will vary.

Salary: $55,000-60,000 annually, inclusive of comprehensive benefits package

RESPONSIBILITIES:

  • Conduct thorough assessments to identify individual needs, including housing, healthcare, mental health, substance abuse, and other support services.

  • Refer individuals to relevant resources and services such as emergency shelters, transitional housing programs, healthcare providers, mental health counseling, substance abuse treatment, job training programs, and legal assistance.

  • Provide crisis intervention support, responding promptly to mental health crises, substance abuse relapses, or threats to housing stability, and offering immediate assistance and support.

  • Assist individuals in navigating the housing process, including helping them complete housing applications, identifying affordable housing options, and addressing barriers to housing access.

  • Collaborate with healthcare providers to ensure individuals receive necessary medical care, including accompanying them to appointments if needed.

  • Develop and implement individualized case plans tailored to each client's unique needs and circumstances, outlining specific steps and timelines for achieving goals.

  • Participate in crisis intervention and screening for psychiatric disorders; respond promptly to crises and to calls from stakeholders involving concerns about clients in need of assistance and complete appropriate documentation and aftercare.

  • Represent Pathways at trainings, community meetings, and other venues as needed

  • Complete tasks and responsibilities as assigned.


QUALIFICATIONS:

  • Bachelor’s degree in psychology, social work, or behavioral health from an accredited university required. Master’s degree in a related field is preferred. LGPC, LGSW, CAC and/or pertinent certification (RBT/harm reduction/certified mental health technician) is preferred.

  • Minimum 3-5 years of experience in homeless outreach, case management and/or counseling of people with psychiatric disabilities, histories of substance abuse and/or trauma, and individuals experiencing homelessness.

  • Demonstrated knowledge of housing services, the DC coordinated entry system, mental health, and substance use disorders.

  • Excellent organizational, oral and written communication, time management, and computer skills.

  • Successful track record of working independently, demonstrating initiative and complex problem solving skills.

  • Bilingual English/Spanish preferred but not required.

  • Driver’s license required.
