Field Case Manager Nurse or Social Worker (LPN or MSW)

A-Line Staffing Solutions is hiring a Field Case Manager (LPN or MSW) in Fort Myers, FL! This is a hybrid/remote opportunity with a managed care Fortune 500 Company. 

Field Case Manager (LPN or MSW)

• Pays: $28 per hour, plus mileage reimbursement 
• Full-time: Monday to Friday, 8am to 5pm
• Hybrid (field/Remote) – making face to face visits at members home or other agreed upon public locale; must be near or local to Fort Meyers or Cape Coral areas
• Contract with potential to convert to permanent based on performance, attendance, and business needs.

Responsibilities:

• Coordinating care for special needs family on the CMS plan
• Works on non-acute members

Performance expectations/metrics:

• Caseload compliance per CMS contract. Field visits, assessments, care plans

Required Skills/Experience:

• Pediatrics experience
• License Practical Nurse with 4 years Pediatrics experience
• Will also consider Masters Degree in Social Work with 1-year working with children/pediatrics experience 
• Software Skills Required: Microsoft Office, Basic Excel skills

Preferred Skills/ Experience:

• Bilingual preferred
• Experience with electronic medical records
