Job Description
Salary: $52,000.00 - $65,000.00 annually

Duties and Responsibilities:

  1. The Care Coordinator Supervisor will coach care coordinators around engagement and completion of a Family Interview with each family.
  2. The Care Coordinator Supervisor will support the care coordinator as needed in the creation of a Child and Family Team.
  3. Have an ongoing awareness of the community resources that could potentially be useful to the child and family and be able to develop positive relationships with these providers in order to ensure access and quality services to the family.
  4. Must be available to the care coordinators during times of crisis.
  5. The Care Coordinator Supervisor assures that care coordinators are utilizing the wraparound process and supporting families in a strength-based way.
  6. The Care Coordinator Supervisor will review and approve Plans of Care.
  7. Plan of Care will be regularly evaluated for adherence to wraparound principles and will utilize the WFAS documents to support increased skill attainment of care coordinators.
  8. Ensure timelines are adhered to including child and family team meetings required monthly.
  9. Will attend CFTs as needed
  10. The Care Coordinator Supervisor will participate in home visits, phone calls, and other support activities between CFTs with care coordinators.
  11. The Care Coordinator Supervisor will carry a caseload determined appropriate by supervisor.

 

Supervision Responsibilities:

  1. The Care Coordinator Supervisor will ensure the timeframes established by the wraparound model are met for all families, including those assigned to their caseload.
  2. Timeframes are as follows:
    1. Contact initiated with the client's family or caregiver within three (3) business days of receipt of the referral.
    2. Conduct initial face-to-face meeting with child and family within seven (7) business days of case assignment.
  3. The Care Coordinator Supervisor will assist in the training of newly hired Care Coordinators, including modeling the process; taking new staff to outside meetings; demonstrating file management and documentation responsibilities and later observing and providing feedback about Care Coordinator's performance during supervision.
  4. Participation in joint supervision with Clinical Program Supervisor as necessary.
  5. The Care Coordinator Supervisor may be asked to represent Wraparound Maryland at community meetings. Professional dress and demeanor is expected.
  6. The Care Coordinator Supervisor will ensure they and staff comply with all required documentation and data collection in support of the program's implementation and evaluation strategy.
  7. Maintain CANS certification
  8. Identify training needs of staff, making Clinical Program Supervisor aware of need and proposed interventions.
  9. Participate in activities that promote personal and professional growth.

 

Qualifications:

  1.  The Care Coordinator Supervisor must possess a Master's Degree in a human services field or equivalent experience.
  2. Must posses a LGPC or LMSW.
  3. The Care Coordinator Supervisor must have two (2) years of experience in a human services position: at least one (1) year of experience working in community-based service provision and at least one (1) year of experience working with children, youth and families.
  4. Possess an understanding of child and adolescent development.
  5. Ability to create effective relationships with individual of different cultural beliefs and lifestyles.
  6. Strong interpersonal skills and the ability to work effectively with a wide range of constituencies in a diverse community.
  7. Effective verbal and written communication skills.
  8. Ability to develop and deliver a case presentation.
  9. Strong organizational skills and detail oriented.
  10. Ability to analyze complex information and to define and solve problems.
  11. Ability to work effectively in a team environment.
  12. Manage time and work effectively with minimal supervision.
  13. Effectively manage multiple priorities simultaneously.
  14. Possess a valid driver's license.
  15. Possess current CPR and First Aid Certification.


Physical Requirements:
• Prolonged periods of sitting at a desk and working on a computer.
• Must be able to lift 15 pounds at times.
• Must be able to access and navigate each department at the organization’s facilities.
