JOB SUMMARY
Manages preadmission process for new referrals, communicates with all contracted hospitals to respond to and addresses urgent and ongoing needs for hospitalized patients awaiting discharge, and provides triage nursing to answer medical questions coming into admissions office and prioritizing the greatest needs for patient visits.
ESSENTIAL FUNCTIONS
Responds to new hospital referrals and information visits with initial telehealth visits.
Makes follow up calls to all hospitalized waiting patients awaiting discharge to ensure all arrangements are prepared for post-discharge visit including medications, durable medical equipment (DME), transportation, and any other needs.
Triages phone calls coming into admissions to answer nursing related questions and prioritize admission visits to ensure patients with the greatest needs are escalated.
Starts charts for new admissions when applicable.
Available to cover admission visits based on staffing and referral volumes.
Conducts in person (out in the community) and virtual preadmission meetings with patients and families including:
- providing patient family education
- entering pertinent information into the electronic medical record (EMR), including demographics, insurance information, and medications.
- communicating with community physician to confirm attending to receive verbal certification.
- collaborating with hospice physician to ensure eligibility.
- sharing pertinent information with respective hospice team
- Sending, receiving, and scanning all necessary preadmissions consent forms in the electronic medical record (EMR).
- Starting the patient chart.
QUALIFICATIONS
Graduate of an approved school of practical nursing. Maintains current Pennsylvania nursing license.
One year of nursing experience as a licensed practical nurse within last four years required.
