Job Description

Licensed Independent Social Worker needed to support clinic in Columbus Ohio. Responsibilities as follows:

'..Census 1,300 patients
Psychosocial assessments – important aspect to overall picture of patient history/ growth
Therapy individual, group sessions
Treatment plans
Development of groups in therapy
Schedule their own clients, referrals from providers, pharmacy, case managers, testing etc.
60 min visits
Multi-disciplinary approach
This is a new program at this site (programs at other locations in US) – will need someone looking to help develop it
Schedule 5-6 patients/day
Charting
Once a month – multi disciplinary team meeting provide updates to providers nationwide & local
Collaborative work with the local clinic, pharmacy and case management teams..'
