Nurse Practitioner - Geriatric - Primary/Wound Care

Join our team of medical providers delivering quality primary and wound care to long-term care facility residents. Impact elderly patients' lives positively!

Responsibilities:
  • Provide primary and preventative care, managing acute conditions
  • Prescribe medications and order/follow-up on treatments
  • Communicate with facility staff and collaborate with primary care physician
  • Perform comprehensive wound assessments, develop care plans, and deliver evidence-based wound treatments
  • Educate patients, families, and caregivers on wound care techniques and prevention strategies
  • Monitor wound healing progress and document interventions accurately in EMR

Qualifications:
  • Current, unrestricted NP license in Kentucky
  • Board Certification and DEA licensure
  • Clinical background in adult, family, or geriatric specialties
  • 1+ years of experience in long-term care, with extensive wound care experience
  • Ability to work with EMR and manage patients with advanced illness
