ABOUT THE POSITION

At Goodwin Hospice, we're seeking a Psychosocial Services Team Lead/Bereavement who will be responsible for supervising licensed and support field team members and directing psychosocial program services, Spiritual Counselors, and Bereavement services. This team member works in collaboration with the Interdisciplinary team to meet the emotional and psychosocial needs of patients and families.

DUTIES

  • Ensures compliance with all state, federal, and CHAP social service regulatory requirements and the Social Work Practice Act.
  • Implements the hospice Bereavement program in accordance with CMS Conditions of Participation and Goodwin House P&P and provides support to facility staff and hospice team.
  • Documents the type of bereavement follow-up offered and ongoing bereavement progress notes as appropriate for up to thirteen months after the death for any family members/POA who receive bereavement support.
  • Supervises the assessment, planning, implementation, and evaluation of social service clinical care by members of the social work team.
  • Provides oversight and coordinates medical social services and activities including psychosocial, bereavement care, hospice counseling and case coordination.
  • Assures proper documentation of social services in clinical records in compliance with state, federal, and CHAP standards. Problem solves computer documentation issues.
  • Actively participates as an Interdisciplinary team member, offering insight and suggestions for care plan updates for patients.
  • Provide backup caseload support when needed.
  • Coordinate and provide clinical supervision in accordance with regulations and requirements of the Board of Social Work examiners and state regulations to designated clinicians who are working towards the LCSW-C licensure, and/or to MSW Interns.
  • Ensure effective management of clinical, administrative and supervisory functions as related to personnel and hospice policies and procedures in order to meet GH mission of patient care.
  • Informs the Administrator of any problems or circumstances, which may compromise the integrity of Goodwin Hospice.
  • Participates in organizational and departmental goals and performance improvement activities.
  • Uses good judgement, discretion and integrity to review, research and resolve problems or complaints in area of expertise.
  • Willingly takes on additional duties and/or assignments, assisting with projects and responding to requests as assigned or as necessary.

QUALIFICATIONS

  1. A clinical social worker who is currently licensed by the Commonwealth of Virginia Department of Health Professional. Minimum 5 years’ experience as a clinical social worker, preferably in a hospice and palliative care setting.
  2. Demonstrated leadership and organizational ability with a motivation to assist in the implementation of program development.
  3. Demonstrated ability to assess and respond to the needs of terminally ill residents and families in varied settings.
  4. Ability to cope with family emotional stress and tolerance of individual lifestyles.
  5. Demonstrated computer skills for documents, spreadsheets and database management and ability to learn new electronic medical record software and become a trainer in health record documentations.
