Job Description

Provides individuals, families, and groups with the support needed to cope with chronic, acute, or terminal illnesses.
Complete diagnostics evaluation, assessments and follow-up with indivdual appointments.
Develop and monitor treatment plans and document weekly or bi-weekly interactions with their client's.
Ability to work from home at least 10 to 15 hours a week.
