General Function: Responsible to the Director for the management of resident assessments, case management, counseling, and treatment. Establishes and maintains agreements and relationships with agencies, as well as an informal network with charitable, religious, and human service groups to establish, deliver and promote family support and readiness, and projects long-term family readiness and support plans and resource needs. Dismas has the right to modify operating standards to include position qualifications and functions to ensure the quality and continuation of services.

Schedule: Full-Time. Mon, Tues, Thurs, Fri 8:00 AM-4:00 PM; Wednesdays 11:30 AM-8:30 PM

Education Requirement: Two years of academic studies in a social or behavioral science program

Areas of Responsibility:
  • Provides Individual Case Management of Facility Residents: Developing, coordinating, monitoring, evaluating, and documenting risk needs assessments, individual reentry plans, goals, and after-care plans that meet the specific needs of each resident.
  • Provides orientations for assigned residents; by providing individual, family, and group counseling. Monitoring and documenting relevant aspects of resident movement, behavior, and progress including but not limited to case notes, reviews, reports, formal and informal disciplinary procedures, urine testing, accountability check.
  • Leads Life Skills group counseling sessions. Conducts and documents substance abuse testing. Performs onsite visits to resident homes and employment. Provides community development and maintains relationships with government agencies and community partners, correctional, medical, employment, and residential homes and employment sites as required.
  • Works assigned schedule. In the event of a work stoppage, work the hours necessary to staff the center. Attends and participates in all required appointments/meetings onsite, within the community, in the court system, or at other business-related locations. Ability to travel overnight for attendance at meetings and training sessions as required.
  • Maintains accurate and timely counseling records by updating the Fresh Start Client Management System and by submitting and documenting all evaluation records, reports, correspondence, and statistical information, as required.
  • Initiates and maintains cooperative and effective liaison activities with industries/businesses, supervising authorities and other agencies, community groups, and professional associations.
  • Conducts initial home checks and monthly home checks for residents as required.
  • Maintains working knowledge of all procedures and policies Statement of Work, Physical Plant, and Operations Manuals, including but not limited to: Emergency Drills, Life Safety, Facility Sanitation, and Escape and Unusual Occurrence Procedures.
  • Communicates all relevant information to supervisory staff regarding residents, facility, and company concerns.
  • Responsible for general housekeeping of their workspace including dusting, polishing, vacuuming, and emptying of trash.
  • Any and all other job duties as may be assigned.

Minimum Qualifications:
  • Two years of academic studies in a social or behavioral science program at an accredited college or university and at least one year of paid-related work experience.
  • Paid work experience in Social Services or Corrections may be substituted for academic studies on a one-year to one-year basis.
  • Ability to read and interpret documents such as assessment reports, safety rules, operating and maintenance instructions, and procedure manuals.
  • Ability to write reports and correspondence.
  • Ability to speak effectively before groups.
  • Ability to calculate figures and amounts such as proportions and percentages.
  • Ability to add, subtract, multiply, and divide in all units of measure.
  • Ability to apply common sense understanding to carry out instructions furnished in written, oral, or diagram form. ability to deal with problems involving several concrete variables in standardized situations.
  • Obtain CPR and First Aid Certification, RAM, FreshStart, Spin (as required), Defensive Driving Certification during initial orientation, and maintain those certifications throughout employment.
  • Must possess a valid driver's license.
  • Must be at least 21 years of age.
  • Experience with Windows and Microsoft Office programs.
