We are looking for like-minded, dynamic individuals to come and join our team.

ABOUT US
The Caring Place provides emergency shelter, residential programs, job training, education & computer literacy classes, healthcare, transitional housing, permanent housing, employment opportunities for homeless men, women, and children as well as programs for “at-risk” youth.

We are currently seeking a FT Case Management Associate at our homeless shelter in Hollywood, FL.

JOB SUMMARY

The Case Management Associate (CMA) reports to the Emergency Services Manager and assists in the intake and orientation of new residents to the program. The CMA will screen clients referred for intake, perform intakes, conduct aftercare, facilitate motivational groups, perform outreach, as well as monitor the Intake/Screening Unit and Support Services. Duties also include crisis intervention and support as needed in the intake area: may facilitate organized and smooth running process of all medical clearances for timely entrance into program.

DUTIES

  • Assist with the monitoring of residents and overnight guests throughout the Center, ensuring that residents are treated with dignity and respect
  • Screens and process intakes as well as conducts orientation for new residents
  • Submits completed intakes to Lead Case Manager or Emergency Services Manager for full review
  • Creates and maintains client files including but not limited to: intake summary, case plans, case notes, aftercare notes, and documentation of discharge planning, maintaining standards of care for contract compliance
  • Completes discharges within timeframe specified by supervisor
  • Performs aftercare and outreach when directed by supervisor .
  • Assists with maintaining and monitoring new and existing residents and facility security
  • When acting as staff on duty, follows all required procedures, guidelines and checklists. Serves as role model for homeless residents in areas such as hygiene, manner of dress, demeanor and attitude.
  • Suggest remedial or corrective actions and assists residents in making better adjustments and in planning intelligent life goals. Corrects residents fairly but firmly, according to BOC guidelines. May offer advice consistent with Program guidelines in areas such as hygiene, dress code, attitude and personal behavior. (However, counseling and discussions of personal, legal and medical issues are the responsibility of the client’s case manager, or counselor.)
  • Covers for drivers, when necessary, to run errands or transport residents
  • Listens to residents, makes decisions based on available facts using wisdom, compassion and according to BOC policy. Calls supervisor if unsure of how to handle a situation.
  • Speaks clearly and courteously to others at all times. Shows respect to residents and maintains confidentiality.
  • Maintains a friendly, professional attitude with all residents while enforcing BOC policies and procedures
  • Assigns tasks and duties to residents and demonstrates, when necessary, how to perform them properly and safely. Monitors assignments to ensure that tasks are completed safely and satisfactorily.
  • Completes all aspects of aftercare and submit aftercare end-of-month report to Supervisor

  • Completes Daily Bed Count Sheets, Dorm Charts, Sign-Out Sheets and Census Totals on weekly basis
  • Will lead and/or participate in devotions, prayers, or other ministry activities ex. outreach events
  • Must be in agreement with and sign The Caring Place Statement of Faith
  • Performs other related duties as assigned

REQUIREMENTS

  • Requires a High School diploma or GED; some college level coursework in social work or demonstrated knowledge of addiction and relapse prevention preferred.
  • Must have a minimum of one (1) year experience in a non-profit organization or general office setting
  • Must have valid State of Florida driver’s license (4 point max)
  • Must be of good moral character with knowledge of homelessness and addiction issues, and have the ability to lead clients by example
  • Requires good communication, computer and organizational skills.
  • Must be able to multi-task and make sound decisions in a fair and compassionate manner.
  • Must have good interpersonal skills to effectively interact with a diverse population and all levels of residents, employees, volunteers and guests.
  • Must be computer literate have experience with MS Office Suite (Word, Excel and Outlook)
  • Must be able to understand, speak, read and write English and carry out oral and written instructions

PHYSICAL REQUIREMENTS

  • Must be able to sit for extended periods, and have the ability to lift and/or carry objects weighing up to 25 pounds
  • Must be able to see, hear and speak, in order to interact with staff and the general public
  • Must be able to move around as needed to perform essential job duties
