About Home Study and Post Release Services
KyUMH works in collaboration with the Office of Refugee Resettlement (ORR) to support children who are placed with caregivers across the state of Kentucky as well potentially contiguous states. The goals of the Home-Study Post Release Services Program (HS/PRS) are to:
  • Assess the safety and suitability of a sponsor before an unaccompanied child is released into a sponsor’s care (Home Study).
  • Provide case management support to ensure the continued safety and stability of children in sponsor’s homes and linking children to resources in the community (Post Release Services).
Required Education and Experience
  • A high school degree
  • One-year of experience working in a social service environment
  • One-year of experience in a professional office environment
  • Fluent in both English and Spanish (written and oral)
Preferred Education and Experience
  • Child welfare and/or case management experience
  • Experience working with Unaccompanied Children, refugee or recently migrated population
  • Associate degree
Job Specific Duties and Responsibilities
  • Engage in direct contact with clients, communicating with them on an at least monthly basis to ensure their continued safety, stability and wellbeing.
  • Delivers post-release follow up services through phone calls, in person and virtual contacts conducted with clients.  This includes but is not limited to maintaining monthly contact with recently released clients for up to 6 months, unless the case is sufficiently stable to close sooner.
  • Accompany case managers to client home visits as needed.  
  • Identify appropriate community resources, ensuring they are culturally and linguistically appropriate.
  • Assist clients to access culturally relevant community services, providing referrals and assistance with completing applications and referral forms.
  • Assist with creating, filing, closing and overall daily maintenance of client records in KYUMH electronic medical record and the UC Portal.  Ensure assigned case records meet documentation standards as defined by both federal and state regulations.
  • Collaborates with the Lead Case Manager and Program Director in drafting and submitting quarterly program performance reports. 
  • Complete required documentation and reports within the defined timelines.
  • Ability to respond effectively to a fast-paced work environment, to include but not limited to, comprehension and implementation of frequent policy updates, procedure changes and data requests.
  • Provide administrative support and assistance in compiling program data and service outcomes information to the Lead Case Manager and Program Director. 
  • Maintain compliance, knowledge and understanding of agency policies and procedures, state and federal regulations, accreditation standards and relevant child welfare topics.
  • Maintain effective working relationships with clients, colleagues, consultants, and community partners/agencies.
Expected Hours of Work
This is a full-time position. Days and hours of work are flexible to support the needs of the program. Although hours are typically Monday through Friday, 9a.m. to 5p.m., the schedule may be adjusted to provide support the case management team during some evenings or weekends.  Flexible work hours to include occasional evenings, weekends, on-site and off-site attendance at meetings. Non-essential for inclement weather.

Travel
Travel, if needed, is primarily local during the business day, although some out-of-area and overnight travel may be expected.
