Your Role:

Embark on a journey as Anova’s dedicated Home Health Licensed Practical Nurse! Elevate patient care within the cozy corners of their homes, while seamlessly blending into our prestigious healthcare brigade. We are looking for a dedicated nurse to work 1-2 patient visits daily 7 days a week. This role is a part-time / PRN position. Reliability and ability to drive is a must!

Key Responsibilities:

· Guided Care: Deliver direct patient care under the supervision of an RN, ensuring each individual feels the comfort of home.

· Medication Mastery: Administer and document medication effects, while closely observing treatment progress and side effects.

· Collaborative Excellence: Foster synergies with healthcare professionals, from therapists to home health aides.

· Knowledge Beacon: Enlighten patients and families about health conditions, while promoting preventive care and staying updated with the latest nursing standards.

· Vital Vigilance: Record patient vitals, medical histories, and clinical notes with meticulous precision.

· Compassionate Caregiver: Support patients' daily activities, administer treatments, and provide emotional stability, becoming a linchpin in their healing journey.

· Family Liaison: Serve as a bridge between patients and their loved ones, ensuring guidance, understanding, and transparent communication.

What We’re Looking For:

· Active LPN license in PA or interstate Nurse Licensure Compact

· CPR certified

· Prior experience as an LPN (Home health background? Major bonus!)

· Gear up: Reliable transportation, a valid driver's license is essential

· Flexibility champion: Be ready for the on-call rotation dance
