Job Description

Pillar Health is hiring for a Licensed Clinical Social Worker to support its Integrated Healthcare model.

POSITION SUMMARY

Position involves providing clinical services and behavioral health consultation to patients in an integrated care setting. The Licensed Clinical Social Worker (LCSW) will promote recognition, treatment, and management of mental health conditions and behavioral components of medical conditions. Potential candidates must be an independently licensed as a LCSW or license-eligible as an LCSW in the state of Wisconsin, with substantial mental health experience--including rapid screening/assessment skills, knowledge of brief behavioral health interventions, ability to manage behavioral health crises, and care coordination skills.

Position functions as a core member of the interdisciplinary care team in the delivery of medical, dental, and behavioral health services in an integrated primary care setting.

ESSENTIAL FUNCTIONS

The list of duties and responsibilities is not all inclusive and may be expanded to include other duties and responsibilities, as management may deem necessary from time to time.

  • Complete brief assessments to diagnose patients with behavioral health symptoms or to identify behaviors that impact their physical and oral health.
  • Develop and implement evidence-based, time-limited individualized treatment plans and assist patients with successful goal achievement.
  • Skillfully provide brief (20-30 minute) interventions to patients.
  • Assist the primary care team in developing care management processes such as the use of guidelines, disease management techniques, case management, and patient education to improve self-management of chronic disease.
  • Work with KCHC’s integrated team (Medical and Dental) to identify, treat, and/or refer patients with substance use disorders.
  • Evaluate crisis situations and apply appropriate interventions.
  • Participate in meetings that support KCHC's integrated health care model to provide comprehensive care for clients.
  • Participate in clinical quality assurance efforts by identifying problems related to patient services and making recommendations for improvement.
  • Thoroughly, accurately, and promptly documents all services rendered in the electronic medical record for every patient.
  • Act as a liaison between referral sources and patients, including evaluating patient’s needs and determining if referrals to other programs or facilities is needed, and follow-up on referrals and patient progress.
  • Other duties as assigned.

POSITION REQUIREMENTS

Education and/or Experience

Master’s degree in Social Work or Psychology. LCSW licensure (or license eligible) in state of Wisconsin. At least one year of clinical experience required. Knowledge and experience using DSM-5. Experience working in a Federally Qualified Health center (FQHC) and/or understanding of Integrated Care Model preferred. Equipped with excellent knowledge of behavioral medicine and evidence-based treatments for medical and mental health conditions. Substance Abuse Counselor education and/or certification preferred.

WORK SCHEDULE

Open availability to work up to 40 hours during clinic operating hours, Monday – Thursday, 7am – 7pm, Friday, 7am – 5pm and rotating Saturdays 7am – 3pm.
