The Opportunity:

We are currently seeking a Behavioral Health Care Manager who is interested in supporting and coordinating the mental and physical health of patients on an assigned patient caseload with the patient's Primary Care Provider and other Behavioral Health Providers.

What you'll likely be working on:

  • Supporting the mental and physical health care of patients on an assigned patient caseload
  • Closely coordinating care with the patient's Primary Care Clinician and other members of the Mindset clinical team including Mental Health Providers
  • Facilitating patient engagement, arranging follow-up care, and documenting patient progress and treatment recommendations in EMR and other required systems
  • Providing patient education about common mental health and substance abuse disorders and available treatment options, as well as recommending self-help resources as indicated
  • Systematically tracking treatment response and monitoring patients (live and asynchronously) for changes in clinical symptoms and treatment side effects or complications
  • Supporting psychotropic medication management as prescribed by the Primary Care Provider, focusing on treatment adherence monitoring, side effects, and effectiveness
  • Providing brief psychoeducational and skills training interventions using evidence-based techniques such as behavioral activation, problem-solving treatment, motivational interviewing, or other treatments as directed by licensed clinician
  • Participating in regularly scheduled virtual caseload consultations with the psychiatric consultant and communicating resulting treatment recommendations to the patient's Primary Care Clinician
  • Facilitating treatment plan changes for patients who are not improving as expected in consultation with the primary care provider, the psychiatric consultant and/or Mindset staff
  • Creating, and maintaining lists of referral resources and facilitating referrals for clinically indicated services outside of the organization (e.g., support groups, employee assistance programs, behavioral health specialty care, substance abuse treatment)
  • Developing and complete relapse prevention self-management plan with patients who have achieved their treatment goals and are soon to be discharged from the caseload.
  • Developing and participating in quality improvement efforts and program evaluation

Education, licenses, and experiences required for this role:

  • Bachelor's or higher degree in psychology, education, social work, public health, nursing or other life science OR coaching certification from an accredited training program (as approved by the NB-HWC, Associated Certified Coach (ACC), Professional Certified Coach (PCC), OR Behavioral Health Support Specialist (BHSS) credential).
  • At least 1 year of demonstrated abilities in case management and behavioral health care navigation and coordination
  • Working knowledge of evidence-based psychosocial treatments and brief behavioral skills training interventions for common mental health disorders, when appropriate (e.g., motivational interviewing, problem-solving treatment, behavioral activation).
  • Basic knowledge of psychopharmacology for common mental health disorders that is within appropriate scope of practice for type of provider filling role
  • Experience with evidence-based coaching, counseling techniques and MH crisis line experience
  • A working knowledge of common mental health and/or substance abuse disorders
  • Ability to work during regular business hours M-F in Eastern Time Zone

  • Has excellent communication and interpersonal skills

This is a full-time, virtual role requiring to be available in Eastern Time Zone.

One Medical is committed to fair and equitable compensation practices.

The base salary range for this role is $70,000 to $83,000 per year. Final determination of starting pay may vary based on factors such as practice experience and patient care schedule.
