HORIZONS HEALTHCARE SERVICES is looking for a Home Health Registered Nurse to join our team for Fairfax County and neighboring towns

• Admits patient to Home Health Services
• Refer patient to another discipline as needed
• Administer nursing care and skills to homebound patients
• Establish patient treatment plans
• Monitor and report changes in patient symptoms or behavior
• Communicate with collaborating physicians or specialists regarding patient care
• Educate patients about health maintenance and disease prevention
• Facilitate referrals to other healthcare professionals and medical facilities
• Maintain accurate patient medical records
• Provide advice and emotional support to patients and their family members

Qualifications:

• Previous experience in acute care or home health(required)
• Familiarity with medical software and equipment
• Ability to build rapport with patients
• Strong problem solving and critical thinking skills
• Ability to thrive in a fast-paced environment

Licensed or registered to practice as an RN in the state of Virginia

Preferably at least two (2) year of experience In-Home Health /acute care as an RN

Valid Virginia Drivers License

We are serving the healthcare needs of our community(NORTHERN VIRGINIA) by providing services needed in order to allow an individual to remain safely and comfortably in their home. The Registered Nurse under physician orders, is responsible for the assessment and treatment of clients needing skilled nursing services in the home setting. The Registered Nurse will provide in-home, one-on-one care to a predominately geriatric population. This home care position will give providers the opportunity to apply their nursing skills while allowing for a flexible schedule. Responsibilities include proficiency in nursing management and treatments. The ability to work independently required. Excellent communication and interpersonal skills essential.

Job Description:

Records clinical/progress notes in the patient's clinical record weekly. Instructs the family in the client/patient's program and evaluates the home environment. Instructs other involved agency personnel in the care of client/patients receiving physical/occupational/ speech therapy. Plans for patient's discharge and discharge summaries and instructions. Some Computer Skills for documentation, Able to drive to Client’s home
