The Position:

The Infusion Nurse provides primary patient care for specifically assigned patients to include assessments, care planning, evaluation and education of patients receiving infusion therapy and/or other nursing services in accordance with the care matrices and best practices in the home or alternate site setting. This role is primarily doing home visits but will be back up to the infusion center. Responsibilities include:

• Provide the majority, if not all of the direct care for all specifically assigned patients including initiation and maintenance of all communications with appropriate care team members
• Evaluate and education patient/care partner to safely administer the home infusion therapy
• Evaluate the home environment where the care is to be provided and identify interventions that may be required
• Develop, in conjunction with other clinical team members, each patients Care Map and evaluate patient's response to therapy
• Appropriately discharge primary patients and complete discharge summary
• Develop and maintain caseload scheduling and communications
• Document patient visit to physician or lab drawing station on calendar

Schedule:

• M-F 8am-5pm, 20 hours a week
• Schedule yourself!
• On-call one evening per month & one weekend every 4-6months & holidays

Requirements

• Excellent IV skills
• RN Degree, BSN Preferred
• Minimum 3 years nursing experience with background in infusion therapy
• Current nursing license for applicable state
• Experience in ICU and/or Cardiac Care preferred
• Excellent customer service skills
• Proficient with Microsoft Excel, Word and Outlook
