Family Nurse Practitioner FNP

We seek a full time (M-Thur) Family Nurse Practitioner FNP to join our esteemed healthcare team in our outpatient Primary Care setting. We are passionate about removing barriers in rural America especially those who have difficulty accessing care. Every patient deserves quality healthcare. In this crucial role, you will be collaborating closely with our dedicated healthcare professionals to promote health equity and resilience. 

Responsibilities:

• Diagnose and treat acute, episodic , or chronic illnesses
• Prescribe all necessary medications and treatments
• Perform comprehensive physical examinations of patients
• Develop and implement patient management policies and procedures
• Facilitate referrals to other healthcare professionals and medical facilities
• Communicate with collaborating physician or specialist regarding patient care
• Provides clinical documentation in the EMR system in an accurate, high quality and timely manner
• Educates patients and/or families on chronic medical conditions, preventative care, and medication adherence compliance
• Completes referrals to case management, nursing, pharmacy and specialists as needed
• Adheres to security and privacy requirements

Qualifications:

• Previous experience in nursing or other medical fields
• Ability to build rapport with patients
• Ability to thrive in a fast-paced environment
• Excellent written and verbal communication skills
• Strong leadership qualities
• BLS 
• Master's in nursing
• Board certification
