Behavioral Health Care Coordinator

Our people-first approach to technology-supported care management delivers a preventative care coordination tool suite to help providers, healthcare organizations and payers improve patient outcomes.
In this role, you will be an extension of the provider to help close the gaps in your patient’s care. As part of our amazing team, you will build a rapport with your patients who are struggling with one or multiple behavioral health issues/needs through monthly calls as you support them through services such as behavioral health screenings related to their mental health needs, health goals, and lifestyle factors. You will work with patients and their providers to develop and implement their individualized care plans, monitor their progress, ensure they are aware of and connected to appropriate community resources, and provide support in maintaining mental health related health goals and wellness needs.In addition, you will advocate for their mental health needs by utilizing active listening skills, investigating patient concerns, documenting appropriately, and communicating professionally with the patient’s care circle to advocate a resolution to those needs.
What you will do
Manage and support patients' mental and behavioral health needs via telephonic conversations
Demonstrate fluency in multiple languages is a plus to facilitate clear and accurate communication with a diverse patient base.
Ensure that daily production goals and quality metrics are met within standards set forth by Cosán Group
Complete individualized patient behavioral health care plans and perform coordination services via our software platform
Gather pertinent information related to patients’ behavioral/mental health needs
Monitor and follow-up with behavioral health related aspects of various chronic conditions and medications
Maintain frequent contact with patients to provide behavioral health support, coordination of care related to behavioral/mental health and manage compliance with the care management programs to increase positive outcomes
Collaborate with Cosan Care Coordinators, providers, and other members of the patient’s healthcare team to endure a coordinated and cohesive approach to patient care
Document all communications with patients, their caregivers, and their providers accurately
Communicate to and from facility or practice care clinicians regarding patient needs and/or life-threatening episodes and to ensure comprehensive care plans are complete and accurate
Adhere to documentation protocols and best practices for daily workflow, escalation of client issues, comprehensive care plans, and internal communications

Demonstrate an ability to develop and build trusting relationships with patients and partner practices with a true passion for caring for patients and guiding them in their health journeys

Must be an empathetic and active listener with the ability to thrive both as a team member and independently in a fast-paced environment
- Demonstrate knowledge of community health and social service agencies and additional community resources
Qualifications you need
Associate degree in behavioral health, psychology, sociology, human services, community studies or related field. Certification in Behavioral Health or Mental Health preferred.
Ability to translate medical information, instructions, and documents as needed to ensure comprehensive understanding
Previous adult in-home, in-facility or remote chronic conditions care management addressing behavioral health needs
Previous experience in behavioral/mental health providing care coordination
Demonstration of fundamental behavioral health knowledge, education, coaching skills, and community resources
Previous experience with using and navigating Electronic Health Records

Location: this is a hybrid role based out of our Cleveland, Ohio office
Compensation Range: $20-$25/hour; based upon experience
Expected hours: 40 hours per week
