Responsibilities
The Licensed Professional Counselor (LCSW / LMFT / LPCC) would be responsible for providing professional counseling services to patients and families including direct counseling services, crisis intervention, and coordination of community services/resources. Performance of duties may be autonomous and self-directed as part of an interdisciplinary team. Duties include, but are not limited to:
- Conducts comprehensive, culturally sensitive psychosocial assessments, develops care plans, and provides counseling and crisis intervention services to individuals and families.
- Coordinates services with appropriate clinic and community resources.
- Provides information and education about health/mental health issues to individual patients and to groups.
- Documents assessment, intervention, and treatment data in medical record.
- Any other tasks deemed appropriate and assigned by FPA
Specific Requirements
- Master’s or Doctoral degree in counseling or a counseling related field from an accredited college or university
- Must be currently licensed to practice counseling services in state of Kentucky.
- Minimum two-three years of experience in providing counseling services and broad background with diverse patient populations, preferably in medical office setting
- Knowledge of professional counseling principles, methodology, and ethics and of human psychosocial development within the family, community, and culture.
- Knowledge of the use of therapeutic relationship to foster patient involvement. Familiarity with brief therapy theories/techniques and therapeutic process. Understanding of techniques for facilitating client motivation to change behavior.
- Knowledge of health and social issues impacting diverse clients and their well-being.
- Ability to address concerns through talk-therapy and by modeling adaptive, sustainable behaviors.
- Promote constructive self-reflection, helpful coping techniques, and nurturing relationships.
- Monitor perceptions about and adherence to suggested treatment
- Skill in crisis intervention

- Skill in case management
- Ability to interact effectively as member of interdisciplinary health care team.
- Ability to identify and utilize community resources.
- Ability to effectively communicate, appropriately, and with diverse patient population.

- Ability to establish and maintain positive, professional relationships with all peers, management and patients
- Physical/Mental demands: Ability to sit, stand up and walk frequently; use hands and fingers to conduct various computer work; lift and/or move loads up to 10 pounds; must possess hearing and speech to communicate in person and over the phone, able to focus in an environment with many distractions,
Occasional stress in balancing multiple demands and in dealing with patients/families experiencing tension.
- Communication: Good communication skills (written and oral)
- Computer literacy: Fine motor skills necessary to operate the computer
- Knowledge of Electronic Health Record preferred
