Job Description

Participates as an active member of the interdisciplinary team, determining the nutrition and dietary goals of participants' care plans and monitoring progress toward those goals.
Completes accurate, timely assessments of participants' nutritional status.
Monitors monthly weight, taking into account factors such as medical diagnosis and drug-nutrient interactions.
Develops nutrition-related treatment plans in conjunction with the interdisciplinary team.
Provides nutrition counseling to participants.

Qualifications:
Qualifications

Bachelor's Degree with major studies in food and nutrition or dietetics with ADA registration and PA licensure (LDN), required.
Minimum of two (2) years experience, required.
At least one (1) year of experience working with older adults, required.
