Admission Concierge/Assistant to The Director of Social Work

BROAD FUNCTION:

Assists the Director of Social work with admissions, transfers, and discharges of residents for the Jewish Home. Assists the Director of Social Work with direct and indirect service and assistance to residents and families in matters affecting their emotional, social, economic and physical needs; maintain effective interaction with the staff of the Jewish Home, residents, community agencies and resources.

MAJOR RESPONSIBILITIES:

Assists with admissions, transfers and discharges of residents to, within and from the Jewish Home.

Assists residents in resolution of initial adjustment, inter-personal relationships, policy, business, financial and special needs problems.

Aides’ residents in emotion problems of physical illness and disabilities, transfers, family relationships, etc.

Maintains contact with resident relatives regarding problems and status.

Maintains appropriate case records and compile statistical reports as required.

Participates in appropriate staff and patient care meetings and consultations.

Assists in representing the Jewish Home as recommended by the Executive Director in community meetings, seminars, etc. as appropriate.

Maintains awareness of new concepts and developments in social work techniques, particularly as applied to gerontology and implement.

Attend all required Inservice Education and training sessions.

Assist with filing.

Other job duties as assigned by the Director of Social Work.

EDUCATION & EXPERIENCE:

High School Diploma or GED preferred. Must have the maturity, patience, and empathy to deal with the elderly and disabled and their families under varying circumstances and stress.
