For more than a century, St. Anne’s Family Services achieves its mission by working together to provide high-quality supportive housing programs, early childhood education, mental health and family-based services to more than 2,100 families annually across Los Angeles County.
The Case Manager is responsible for providing services to meet the resident educational, mental health, and medical needs.
Responsibilities:
Develops, creates, and implements goals and objectives of residents
Documents resident’s progress
Develop a self-sufficiency plan with each resident that describes objectives, action steps, timeframes, and supportive services necessary to achieve the objectives
Assist residents with the implementation of the self-sufficiency plan by meeting formally once a week with each resident in their home and informally as often as necessary
Continuously assess residents to identify changing needs and modify their self-sufficiency plan and staff interventions as necessary and plan for discharge concurrently
Active participant in resident’s Child and Family Team meetings
Coordinates and manages information provided by other school professionals
Provides supervision, protection, and care of students individually and in groups at all times
Provides assistance to each student in working with a group and in handling individual problems
Administer discipline and setting of limits for behavior
Assist intake coordinator with intake responsibilities including screening telephone referrals, reviewing intake packets, arranging and conducting pre-placement visits and interviews, determining admission decisions, and completing all necessary admission paperwork if needed
Provide Intensive Care Coordination and Intensive Home Based Services to residents placed in STRTP
Requirements:
Bachelor’s Degree in Social Science, or related field
Must possess and maintain a valid California Driver’s License
Physical ability to perform Pro-ACT, administer first aid/CPR and operate an agency motor vehicle
One year similar experience
Ability to receive and implement constructive feedback.

Employees are expected to maintain composure during all interactions and crisis situations and utilize crisis communication to deescalate client crisis and prevent any serious bodily injury or harm
