Position Summary:

This position is responsible for face-to-face office visits for patients with chronic and/or acute conditions as defined by the White Bird Clinic Nursing Protocols and for coordinating care with a select panel of high risk/ high services utilizing patients with a focus on self-management and improvement of the patient's medical conditions.


Position Responsibilities

Conduct comprehensive physical, mental, and psychosocial assessment.

Develop care plans to prevent disease exacerbation, improve outcomes, increase patient engagement in self-care, decrease risk status, and minimize hospital and ED utilization

Utilize a variety of strategies to collaborate with patients to develop plans that support self-management to optimize their health

Adhere to standing orders and nursing protocols to order labs, referrals, medications, and provide education to support patient health improvement goals

Provide and update comprehensive care plans

Coordinate specialty care including assisting patients in navigating the health care systems and services, behavioral health services, and substance treatment services

Partner and collaborate with internal White Bird and external case management programs to coordinate care

Define, develop, and maintain a high-risk patient panel

Utilize data to engage in quality improvement related to the defined high-risk patient panel

Document all patient communication and care provided in EMR that aligns with the documentation requirements for Meaningful Use and other clinical benchmarks required by our grant funders and clinic accepted initiatives

Participate in the continuous improvement of procedures, protocols, policies, and related materials related to providing face to face visits and managing a high-risk patient population panel

Required Qualifications

Current Oregon RN license with a bachelor's degree or higher

Ability to work with a diverse population and a broad range of situations while maintaining a professional and courteous attitude

Willingness to participate in continuing education

Minimum of five years nursing experience or specialty certification

Experience in the delivery of primary care

Ability to work effectively with minimal supervision

Strong computer skills including but not limited to the ability to send and receive email, write documents in MS Word, lookup information using the internet, and use the electronic EHR system

Maintain current BLS and complete annual blood borne pathogen training

Receive approval of the Medical Director showing complete knowledge of all the clinic's Acute and Chronic Care Nursing Protocols within six months of hire date

Complete White Bird new employee training

Able to function in a chaotic environment

Preferred Qualifications

Bilingual in English and Spanish

Experience working in a consensus environment

Minimum of two years' experience in primary care medical practice

Display knowledge of Chronic Disease processes including but not limited to diabetes, hypertension, asthma, COPD, hyperlipidemia, and the management of Warfarin

Compensation Details:

This is a non-exempt position with target starting base compensation ranging between $45-$48 per hour, paid bi-weekly.

In addition, this position qualifies for a competitive health and welfare package and generous paid time off opportunities.
