Job Description

Do you have an Associates level degree in a human services field of study or 3 years of experience in the developmental disabilities service delivery system, communicating in the primary language of the individual and their family, and knowledge of the regional center system.

Remarkable & Centered Supports uses a Person-Centered approach when interacting with individuals and their families. Services are tailored and individualized to meet the needs of each person to support them in developing and living a quality of life that works for them.

Hours worked are provided to individuals and their families between Monday through Friday 8am-8pm.

Job Title: Coordinated Family Supports Mentor

Job Summary:

The CFS Mentor supports and provides direct service for a caseload of individuals receiving Coordinated Family Services. The CFS Mentor supports individuals receiving service and their families in connecting and applying for any needed generic resources such as SSI, Medi-cal, CalFresh, ACCESS, IHSS, and accessing and dealing with any government agencies. This position reports directly to the CFS supervisor.

· Travel throughout various cities, Downey, Lakewood, Bellflower, Norwalk, Long Beach, Carson, Torrance, Lomita, San Pedro.

. Knowledge of the regional center system and experience working with individuals with intellectual disabilities, preferred.

· Complete daily CFS daily notes and any other documentation.

. Implementation of Individual goals as outlined by the circle of support.

. Understand, practice, and implement the CFS Philosophy.

. Provide support to individuals in daily living routines.

. Supervise and teach individuals in self-help skills, social skills, advocacy skills, and community skills.

. Reflect agency policy in all interactions with individuals. Have positive/caring behavior interactions with individuals and staff.

. Attendance and participation at team meetings and in-services is

required of employment.

. Responsible for documentation of objective progress, individual

progress, medical follow-through, special incident reports or any other documentation.

. Has knowledge and follows agency safety procedures and disaster plans.

. Responsible for medical follow-through as designated by physicians.

. Responsible for assisting individuals with accounting of funds, filing receipts.

. The direct care staff directly reports to the CFS Program Supervisor.

· Establish and Maintain relationships with employment agencies, partnerships, regional centers, and other resources.

· Advocate for person served and request reasonable accommodations if needed.

· Performs other duties as assigned.

Qualifications:

· Minimum qualifications for CFS staff include an Associates level degree in a human services field of study or 3 years of experience in the developmental disabilities service delivery system, communicating in the primary language of the individual and their family, and knowledge of the regional center system.

  • High School Diploma or equivalent.
  • At least eighteen years of age.
  • Pass criminal background clearance through the Department of Justice, Federal Bureau of Investigation and Child/Adult Abuse Index.

Salary Range:

· $19.00/hour - $20.50

Job Type: Part-Time & Full-time hours available 35-40 hours a week.

Schedule:

  • 6-8-hour shifts
  • Hours worked are provided to individuals and their families between 8am-8pm.
  • Monday to Friday, some Saturdays as needed.

Experience:

Knowledge of the regional center system and experience working with individuals with intellectual disabilities, preferred.

Experience working in Independent and Supported Living Services preferred.

Ability to commute/relocate:

  • Long Beach, Lakewood, Cerritos, Norwalk, Signal Hill, Bellflower, Carson, Lomita, Torrance, San Pedro, and other nearby cities: Reliably commute or planning to relocate before starting work (Required)

Language:

  • Bilingual (Preferred) Speak other languages: Spanish, Tagalog.

License/Certification:

  • Driver's License (Preferred)
  • First Aid & CPR

Willingness to travel:

  • 75% (Preferred)
