Yuvo works to increase access to primary care by supporting federally qualified health centers (FQHCs), the institutions that have dedicated the most to care for underserved communities. We work with FQHCs to help scale the quality of care they provide by providing access to new payment systems, developing and optimizing operational infrastructure, and convening partnerships in the broader care ecosystem so that they can increase their capacity and deliver the highest quality of care to their communities.

About The Role

Join our team as a Care Navigation Specialist, where you'll take on a crucial role in outreach and providing tailored support to clients navigating the complex healthcare system. Reporting to the Director of Care Management, your responsibilities will include contributing to care management program development, providing individualized support and assistance to clients in navigating the healthcare system, and assessing community resources. We're seeking a self-starter with a solution based attitude who is ready to make a meaningful contribution to the overall healthcare of our patients.

This is a hybrid position with work in and around New York City.

This role will require regular travel to New York City based FQHC partners.

At this time, Yuvo Health is not able to provide visa sponsorship or work authorization support.

What You'll Do
  • Perform outreach activities to engage patients and identify their health and social needs.
  • Conduct comprehensive assessments of patients needs, including health, social, and behavioral.
  • Develop individualized care plans in collaboration with the patient, healthcare providers, and other stakeholders.
  • Provide ongoing support and guidance to patients in accessing healthcare services, scheduling appointments, and understanding treatment plans.
  • Coordinate with community resources and social service agencies to address patients' non-medical needs, such as housing, transportation, and financial assistance.
  • Co-design our community resource directory.
  • Advocate for patients rights and preferences within the healthcare system, ensuring they receive culturally competent and patient-centered care.
  • Maintain accurate and up-to-date documentation of patient interactions, assessments, and care plans.
  • Monitor patients' progress and outcomes, and adjust care plans as needed to achieve desired goals.
  • Participate in team meetings, case conferences, and training sessions to enhance knowledge and skills.
  • Collaborate with interdisciplinary team members, including nurses, social workers, and care coordinators, to provide seamless care coordination and support.
  • Adhere to all regulatory requirements, ethical standards, and organizational policies.
  • Evaluate effectiveness of outreach efforts and interventions through data collection, analysis, and feedback from community members.
  • Articulate findings to the broader team in efforts to scale program components.
  • Perform any other duties as reasonably required by the Company.
What You'll Bring
  • Bachelor's degree in social work, public health, nursing, or a related field (preferred).
  • Previous experience working in healthcare, social services, or community outreach.
  • Knowledge of healthcare systems, resources, and community services.
  • Strong interpersonal and communication skills, with the ability to build rapport and establish trust with clients.
  • Cultural competence and sensitivity to the diverse needs and backgrounds of clients.
  • Excellent organizational and time management skills, with the ability to prioritize tasks and meet deadlines.
  • Proficiency in using electronic health records and other relevant software applications.
  • Ability to work independently and as part of a team in a fast-paced environment.
  • Valid driver's license and reliable transportation (if applicable).
  • Certification or licensure as a Community Health Worker or similar credential (preferred).

Base Salary Range: $75,000 - $85,000/year
