Licensed Practical Nurse (LPN)

Clinity Talent is actively hiring RN's and LPN's to work with our exclusive partner in the Roxbury, and Boston, MA area. This non-profit organization is dedicated to providing the best care for our elderly homeless population. The resources they offer to their clients/patients is amazing. We are seeking multiple Registered Nurses (RNs) in the Greater Boston Area who are interested in permanent Case Management roles to start ASAP. These are NOT remote positions. Must be onsite. New graduate RNs are welcome to apply, no experience required! We are open to part-time candidates as well (minimum 25 hours per week).

Responsibilities for this position include:

• Review of care plans, and participating in assessments
• Developing assessments frequently
• Discharge planning.
• Updating service plans
• Collaborate with interdisciplinary team.
• Supervising CNAs

What we require from you:

• Associate in nursing, or BSN.
• MA RN licensed.
• Passion for helping people.
