Job Description:

• Seeking an Experienced Home Health Nurse with 1 year of home health nursing experience.
• Proficiency in Axxess software and excellent charting skills required.
• Critical role in providing high-quality care and accurate documentation.

Key Responsibilities:

• Conduct comprehensive health assessments in clients' homes.
• Develop individualized care plans for optimal outcomes.
• Administer medications and skilled nursing care.
• Monitor patient progress and adjust care plans accordingly.
• Provide patient and caregiver education.
• Maintain accurate electronic records using Axxess software.
• Effective communication with patients, families, and healthcare professionals.
• Ensure compliance with home health regulations.

Qualifications:

• Minimum 1 year of home health nursing experience.
• Proficiency in Axxess software with at least one year of hands-on experience.
• Strong charting and documentation skills.
• Current nursing license in Illinois.
• Excellent communication and interpersonal skills.

• Reliable transportation and valid driver's license.
