Coordinated Care Services Inc (CCSI) seeks a talented Community Case Monitor II to contribute their expertise in providing essential business services to our provider organizations and customers. The Community Case Monitor II will be located in Onondaga County and is responsible for all aspects of monitoring Assisted Outpatient Treatment (AOT) Enhanced Agreements, AOT Court Orders, and Forensic Transition Cases. The position will provide support and oversight for high-needs individuals with serious mental illness who have a history of medication non-compliance and who live in Onondaga County. The Community Case Monitor II will coordinate and facilitate multi-disciplinary teams that include: the client, behavioral health treatment providers, case management, and housing supports, as outlined in the treatment plan. CCM II will support the data collection and reporting requirements of this program. CCM II will assist in supporting the evaluation process for CPL Article 730 orders for individuals residing in the community. Supervision will be provided by the Manager of Forensics, in close collaboration with the AOT Coordinator.

CCSI is partnering with Onondaga County Adult Mental Health to hire a Community Case Monitor II. This position is employed by CCSI and working on behalf of Onondaga County Adult Mental Health.

In this role, you will

  • Collaborate and assesses needs of individuals referred to and assigned by the AOT Coordinator and the Manager of Forensics.
  • Coordinate the development of system-wide treatment and support plans to include treatment services, case management, housing, care management, other identified supports.
  • Monitor client's participation in and compliance with established treatment plan.
  • Schedule, plan and facilitate team meetings.
  • Communicate effectively with the client, providers, families and criminal justice personnel to assist with developing and sustaining the plan.
  • Create a sustainable treatment team by assigning roles and tasks to the treatment team to prepare for the transition to a lower level of care.
  • Monitor treatment implementation and coordinates action when treatment, service and support plan needs adjustment.
  • Recommend adjustments to treatment plans, and advocates for system movement.
  • Track individuals across multiple systems of care over time to assure continuity of linkages and supports.
  • Work closely with treatment providers, ACT, case management, housing providers, parole, probation, Peer Bridger, and unique services and supports.
  • Cultivate partnerships with community providers to best support clients.
  • Receive referrals of persons allegedly in need of Assisted Outpatient Treatment and coordinates investigations.
  • Support and facilitate the CPL Article 730 Order process as needed.
  • Support community education requests.
  • Request and review clinical records.
  • Culturally responsive to the population being served in the development, design, monitoring, evaluation and/or implementation of programs and services.
  • Organize interviews or meetings with applicant and providers involved in the plan as needed.
  • Attend staff and other community meetings.
  • Maintain and organizes electronic and paper records, updates tracking sheets. Records progress notes in a comprehensive, timely manner.
  • Support clients in achieving their program goals.
  • Develop, track, and report program goals and objectives.
  • Travel as necessary to fulfill responsibilities.
  • Mentor other staff as needed or requested.
  • Support set up, facilitation, and execution of community based 730 examinations.
  • Identify creative solutions to complex problems utilizing available community resources
  • File maintenance for the program, including filing and archive of records.
  • Delegated supervisory task as needed by Manager of Forensics
  • Perform other duties as assigned.

  • A working knowledge of laws and regulations regarding the treatment of individuals with a serious mental illness.
  • A working knowledge of systems of care (i.e. OMH Housing, Chemical Dependency, Criminal Justice, Homeless, Health Care).
  • Ability to prepare effective written correspondence, records, and reports.
  • Good powers of observation, analysis, and negotiation.
  • Ability to work in a person-centered way with clients, families.
  • Ability to work collaboratively with individuals, families, law enforcement, the court system, state agencies, state forensic services, and local service providers.
  • Ability to facilitate multi-disciplinary team discussions or meetings.
  • Ability to work with minimum direction and supervision.
  • Ability to plan, schedule and facilitate complex cross systems meetings.


Requirements

Education and Experience
  • Bachelor's degree in Human Services field plus four years of experience in mental health required, with an additional one year of experience in the community case monitor 1 position.

  • Must have a valid NYS driver's license and reliable transportation.
  • Satisfactory completion of all required background screenings: criminal, and driving.
