Job Description

Join our client's team as a full-time Hospice Registered Nurse (RN) in Jersey City/Newark, NJ. This role involves providing comprehensive end-of-life care to patients and families facing terminal illness. Responsibilities include utilizing nursing expertise to implement prevention, therapeutic, and rehabilitative care and ensuring patient safety.

Qualifications:

Required:

Associate degree or higher
Over one year of experience in any of the following areas:
- Home Health
- Medical-Surgical (Med-Surg)
- Community Health
- Hospice
Valid Registered Nurse (RN) license in the state of New Jersey (NJ)
Valid American Heart Association Basic Life Support (AHA BLS)

Preferred:

Bilingual in Spanish
RN License Number
Specialized certifications

Key Responsibilities:

Provide direct or indirect skilled comprehensive nursing care
Independently deliver end-of-life care to patients and families
Utilize nursing process for data collection, planning, and evaluation
Administer medication, record vital signs, and reinforce patient education
Communicate effectively with healthcare team and report changes in patient status
Maintain a safe environment for patients and self
Identify and address learning needs
Adhere to company policies and procedures
Utilize EMR: Home Care Home Base and provided equipment: Tablet, Phone
