Job Description

Summary: 

The Patient Care Coordinator (PCC) serves as a liaison between patients, their families and healthcare professionals.  The PCC work closely, as a team, with the provider(s) to meet the needs of the patients and follows approved guidelines, protocols, and standing orders.    

ESSENTIAL DUTIES AND RESPONSIBILITIES:

  • Plan, coordinate and carry out patient care functions.
  • Address patient requests and needs in a professional manner.
  • Respond to patient concerns timely and ensure patient satisfaction.
  • Inform patients of normal/abnormal labs, diagnostics
  • Schedule urgent care patient appointments following physician’s orders
  • Schedule diagnostic testing and procedures for patients at hospitals and our clinic, making sure patient understands any instructions regarding preparation for procedure
  • Scheduling referrals, as directed
  • Collaborates with pharmacies and provider(s) to ensure accuracy and safety of medication
  • Knowledgeable on Community Resources
  • Facilitates care with transportation services, home health agencies, physician offices, equipment companies, and nursing homes
  • Maintain current license and certification if applicable
  • Coordinate Pre-Authorization process
  • Suggest process improvements to enhance patient services.
  • Documents all patient communication(s) in EMR, appropriately
  • Maintain patient records confidential and secure.
  • Complete PHI log per policy
  • Perform all other duties as assigned

  • Team members must demonstrate skill and knowledge related to effective communication with vulnerable patient populations.
  • Team members must participate in Continuous Quality Improvement activities within the organization to ensure patients receive high quality care.
  • All team members will be involved in the process of improving quality incomes.
  • Team members will participate in the review and evaluation processes of practice performance and help to identify opportunities for improvement.
  • Team members will participate in Grace Health's advocacy program.

GENERAL DUTIES:

  • Follows policies and procedures of the office, including administrative, clinical, quality assurance, and personnel
  • Maintain good attendance (daily, meetings, and other assignment tasks)
  • Maintain timely documentation of all work assignments
  • Maintain patient confidentiality
  • Routinely keep supervisor informed about attendance and job assignments
  • Flexible in being able to multitask
  • Work effectively and at an efficient pace
  • Work cooperatively with physicians, administration, and peers

EDUCATION and/or EXPERIENCE: 

Must have high school diploma and significant medical office experience, certification as medical assistant, or nursing license
