Responsibilities:

🩺 Perform comprehensive patient assessments and develop care plans.
💉 Administer medications and conduct treatment as prescribed.
🌡 Monitor patient conditions and adjust care plans as necessary.
📋 Maintain precise medical records and documentation.
👥 Work collaboratively with healthcare professionals to ensure complete patient care.

Qualifications:

🎓 Nursing degree with active RN or LPN license in Connecticut.
♥ Current CPR/BLS certification.
🏡 Prior home health experience is a plus.
