We are actively seeking to add a qualified physical therapist to our current incredible group of team members.

If you have the relevant skills and experience, are currently licensed in Texas or in the process of obtaining your Texas Physical Therapist license, and are looking for a great place to work in an outpatient physical therapy setting with tremendous people to work with every day, we would like to meet you! Responsibilities:

• Complete and comprehensive evaluation of patients in an outpatient setting
• Establishment of comprehensive Plan of Care to address identified deficits and restore prior level of function
• Collaborate and communicate with Clinic Director and Team members to ensure continuity of care
• Assess patients regularly for appropriate progression and/or modification of POC
• Promote a positive work environment and contribute to our team approach
• Timely, thorough and accurate documentation of patient care provided in our EMR system
• Actively demonstrate adherence to ProActive's Core Values
• Communicate effectively with patients and their family members to promote their active participation in therapy
