Coastal Health & Wellness is seeking an Enabling Services Manager!

The right candidate will be responsible for development, implementation, monitoring and reporting on initiatives that support Enabling Services; case management, benefit counseling or eligibility assistance, health education and supportive counseling, interpretation, outreach, transportation, and education of patients and the community regarding the availability and appropriate use of health services. In addition, is responsible, for the management, training, and coordination of all program activities and staff for the county indigent program, outreach, and enrollment into the federally run exchange marketplace plans, and community outreach services to CHW Clinic patients.

Required:           

  • Bachelor’s degree in business, management or a related field preferred.
  • Graduation from an accredited high school or equivalent.
  • 3+ years supervisory experience required.
  • 3+ years of progressively responsible healthcare management experience preferred.
  • One year’s program management, health maintenance program development or related experience preferred.
  • Knowledge of general office procedures; insurance plans; and various federal and state programs.
  • Bilingual in Spanish preferred.
  • Ability for spur of the moment transportation without relying on organization vehicles.
  • Must possess or ability to readily obtain a valid driver’s license issued by the State of Texas for the type of vehicle or equipment operated.
  • Department of Motor Vehicle check may be required, if applicable.
  • Customer Service Oriented.
  • Excellent written and verbal communication skills.
  • Excellent Computer Skills MS Office.
  • Must be willing and able to work evening and weekend hours if necessary.
  • Must be in compliance with GCHD Immunizations policy.
  • Must be in compliance with ICS training requirements.
  • Must pass criminal background check and drug/alcohol screening.


An equivalent combination of education and work experience which appropriately demonstrates the knowledge, skills and abilities to perform the above described essential functions will be considered when hiring for this role.

 

Traditional Duties:

  • Oversees and manages all activities of Enabling Services teams. 
  • Oversees Enabling Service functions such as case management, benefit counseling or eligibility assistance, health education and supportive counseling, interpretation, outreach, transportation, and education of patients and the community regarding the availability and appropriate use of health services.
  • Formulate and implement operational strategies and initiatives in the Enabling Services department.
  • Provide targeted services through assessment of patient and client needs, ensure resource alignment, and update appropriate stakeholders on strategies, initiatives, outcomes, and issues.
  • Oversees and manages activities within Case Management and Referral programs.
  • Provide targeted case management services through assessment of patient needs and make appropriate social services referrals to outside agencies as indicated.
  • Develops Community health Workers and Supervisors, providing them with training, and ensuring they are versed in how to use customer relationship strategies.
  • Assists in planning, developing, implementing, and maintaining policies and procedures for operation of the Indigent Care program, ensuring adherence to established federal, state, and local laws, rules, regulations, policies, and procedures.
  • Oversees the determination of eligibility for new applicants and continual eligibility for clients for Indigent Care services, reviewing and verifying data on applications, and ensuring adherence to regulations, policies, and procedures.
  • Oversees the interview applicant process to determine eligibility, inquiring as to financial status, living arrangements, medical conditions, etc., and contacts various human services agencies, financial institutions, credit bureaus, landlords, etc., to confirm information received.
  • Ensures adherence to established laws, regulations policies, procedures, and standards.
  • Confers with representatives of other governmental or private human service agencies or health care professionals to coordinate referral of program clients for needed services.
  • Performs monthly quality assurance review and audits on county indigent program enrollment, prepare/send report, and review with staff the findings.
  • Establishes relationships with community organizations throughout the service area and coordinates dissemination of health/ insurance enrollment information.
  • Establishes and manages external partnerships with community organizations/members, businesses, political leaders, and neighborhood initiatives.
  • Works with external partners to plan and implement collaborative population health strategies. Coordinates outreach for Center programs and services.
  • Develops and maintains schedule of community educational events related to enrollment, senior services, and community outreach.
  • Ensures statistics on the overall utilization and benefit of outreach programs are reported. Conducts performance reviews and provides input on direct reports for human resource decisions such as hiring, promotions and disciplinary actions.
  • Serves as liaison to external events, boards and convening groups. Tracks Center guest activity and enrollment numbers, develops, and presents Center status and impact reports to leadership and community partners, as needed.
  • Performs monthly, quarterly, and yearly quality assurance review and audits on departmental functions, prepare/send reports, reviews, referrals, OEE, PAP, with staff the findings.
  • Develops and manages external partnerships with community organizations/members, businesses, political leaders, and neighborhood initiatives.
  • Maintains schedule of events for enrollment in various federal insurance programs.
  • Performs other duties as assigned by supervisor.
