Hybrid or Remote - RN Care Manager - NYS Registered Nurse - MLTC/MAP

Compass Healthcare Consulting & Placement is conducting a search for experienced NYS RN Care Managers for MLTC or MAP Case Management. Qualified candidates will have prior MLTC or MAP RN Care Management experience. Fully Remote and Hybrid positions available, doing telephonic Case Management. Must have NYS RN and reside in NY, NJ or CT. Bilingual Spanish, Bilingual Cantonese, Bilingual Mandarin, languages a plus!

RN Care Manager Responsibilities:

Reporting to the Supervisor of care management, this position will provide telephonic case management to our members including medical, social, psychological, physical and spiritual needs by developing, implementing and monitoring the member's care plan in conjunction with the PCP, caregivers and other team members in order to help meet the member's needs. managed care nurse, managed care rn, nursing, rn, nurse, care management

Qualifications:

• NYS RN license required

• CCM preferred

• At least 1+ years of RN experience in managed care, home care, acute care or nursing home preferred

• Bilingual Chinese (Cantonese/Mandarin), Bilingual Spanish a plus

• Prior experience in an interdisciplinary service delivery environment preferred. Working knowledge of Medicare and Medicaid regulations

Excellent Salary based on relative experience and great Benefits! Salary Range $100,000-110,000
