CSW for Post Incarceration

HopeWorks is looking for a CSW to provide intensive Case Management for Clients with severe mental illness and substance use disorder.

REPORTS TO: Program Manager

ESSENTIAL FUNCTIONS:

Initiate and maintain a therapeutic alliance with CRT Client. Use this alliance to help Clients achieve treatment and life goals with the ultimate goal of Client independence from HopeWorks CRT services.

Complete initial program paperwork with clients. To be completed within 30 days of entry and updated every 90 days.

Complete Client assessments such as: Addiction Severity Index, Symptom Distress Scale, and Mental Status Assessments as needed per CRT policies.

Review and have each client sign a Participation Contract.

Complete strength-based quarterly Treatment Plans with Clients. Treatment Plan goals can and will pertain to any part of a Client’s life, and are not limited to psychiatric or substance abuse goals.

Link Clients to necessary psychiatric, substance abuse, medical, dental or any other services in the community which Client and CSW agree upon.

Teach Clients to advocate for themselves and to obtain community services as appropriately requested by Client.

Update HIPAA paperwork and release of information on clients.

Administer drug and/or alcohol tests in order to set a baseline for Client’s recovery from substance abuse. Such tests will be administered per HopeWorks Behavioral Health agreements with Client.

Assist Clients in identifying actions they may take in managing their mental health symptoms and substance abuse issues. Assist Clients with putting these actions in practice.

Complete Discharge Plan with Client. To be completed yearly.

Complete yearly Crises Plans with Client.

Facilitate group meetings as appropriate.

Prepare periodic written assessments of programs for clients.

Prepare notes and enter billing in a timely manner and in accordance with agency and program guidelines.

Must bill Medicaid at least 300 units per month.

Conduct 60% of appointments in vivo to assess levels of functioning and follow up client medication compliance.

Assist clients with life skills, including financial literacy, transportation and other assistance as needed within the boundaries of CCSS.

Assist with entry into day treatment, psychosocial rehabilitation, vocational, volunteer or educational programs.

Complete Prior Authorization forms for Value Options as appropriate.

Complete Peer Evaluations when requested.

Must participate on a closing team at least one night during the week.

OTHER RESPONSIBILITIES

Carry cell phone and provide crisis intervention as required.

Carry out all duties in a manner that is consistent with HopeWork's mission, vision, values, and policies.

Duties may be changed based on agency needs.

EDUCATION, EXPERIENCE AND LICENSURE REQUIREMENTS

Bachelor’s degree, preferably in human services related field.

At least one (1) year experience in case management or direct care working with SDMI population or other relevant experience.

Current CPR/First aid certification or ability to obtain within 3 months of beginning employment.

Current Valid driver’s license and vehicle insurance.

OTHER REQUIREMENTS

Computer literate including skills in Word and Excel and database programs.

Excellent oral and written communication skills.

Good organizational skills.

Job Type: Full-time

Pay: From $18.90 per hour

Expected hours: 40 per week
