Essential Duties & Responsibilities: This is an advanced Public Health and specialized care nursing position that is responsible for providing comprehensive nursing care coordination services to pregnant women and their families eligible for Nurse Family Partnership (NFP). Responsible for providing knowledge, encouragement, linkages and support necessary to maximize families’ health, well-being, and self-sufficiency. This position is responsible for striving to assure each participant’s continued involvement in prenatal and child health care as well as other needed community services, through actively engaging the participant and building on the families’ strengths, assets and goals. This position conducts home visits, outreach, and plans and coordinates health care services for identified high risk participants preferably via face-to-face visits to assist with the continuity of care. The RN Specialist is responsible for maintaining the highest standards in clinical nursing practice and adherence to the NFP model, and to policies, procedures, guidelines and standards of NFP.

Specific Activities:

  • Provides home visits to women and their families in accordance with the Healthy Start NFP model and guidelines and the nursing process.
  • Carries a case load initially of up to 25 clients with the expectation that after delivery, the infant case will be referred to the Nurse Family Partnership program.
  • Develops therapeutic relationships and utilizes concepts of reflection, evidenced based interventions and motivational interviewing with women and their families in a home visiting environment.
  • Assesses physical, emotional, social and environmental needs of women and their families as they relate to the NFP domains.
  • Assists women and their families in establishing goals and outcomes.
  • Provides education, support and referral resources in assisting women and their families in attaining their targeted goals, including family planning and smoking cessation.
  • Coordinates service delivery and caseload management in a manner that addresses the immediacy of the participant’s needs and identified risks to improve birth outcomes.
  • Understands, supports, and coaches others in the NFP vision, mission and model. Represents the NFP vision, mission and model in actions and verbally to both internal and external customers and colleagues.
  • Creates, maintains, and supports a safe environment for open discussion. Maintains confidentiality of client information and an electronic medical record.
  • Records data according to Healthy Start Coalition of Orange County and NFP requirements as requested.
  • Consults and collaborates with other professionals involved in providing services to woman and families and formulating nursing diagnosis based on nursing assessment and client’s goals.
  • Schedule joint home visit with NFP nurse supervisor every 4 months.
  • Documents case management activities and accurately codes direct services, service data, education, interventions and other pertinent information in participant’s electronic record and other applicable required databases.
  • Completes and submits all administrative documents.
  • Compiles and reconciles data and other information into reports as required.
  • Establishes and records progress toward annual goals.
  • Participates in quality improvement efforts.
  • Participates in review and analysis of reports for achievements, areas for improvement and quality improvement efforts.
  • Performs a range of associated administrative functions, as appropriate, such as completing administrative and programmatic documents, reports and correspondence as required.
  • Attends and completes NFP and Department of Health required meetings and trainings, including but not limited to, week long training in Denver, Colorado for face-to-face education session.
  • Attends and participates in one-on-one meetings with supervisor, bi-weekly case conferences, and team meetings.
  • Actively engages in professional development to meet all NFP nursing competency requirements. Motivated to utilize computer for distance learning.
  • Stays informed of current health care developments to provide safe, quality nursing home visiting services.

Other:

  • Works as part of team to support the Healthy Start Coalition and NFP model.
  • Consults and collaborates with other professionals involved in providing services to women and families.
  • Participates in community coalition in support, and public awareness promotions of NFP. Develops and maintains community relationships to support client referrals.
  • Supports policies, procedures, NFP visit guidelines and standards.
  • Abide by all Healthy Start Coalition of Orange County policies and standards of conduct
  • Perform other duties as assigned by the Executive Director

Education and/or Work Experience Requirements:

  • Bachelor’s degree from an accredited college or university with a major in nursing.
  • Current licensure as a Registered Professional Nurse in accordance with Chapter 464 Florida Statutes or eligible to practice nursing in accordance with F.A.C. 210-6.27(in good standing).
  • Two years of verifiable professional experience in maternal child health, public health, home visiting, mental or behavioral health.
  • Professional experience in conducting home visitations with pregnant and/or parenting women or infants.
  • Must have a valid Florida driver’s license with reliable transportation and a good driving record

  • Must be able to work under pressure and meet deadlines, while maintaining a positive attitude and providing exemplary leadership.
  • Must be able to travel to attend conferences, training and other events, as required.
  • Ability to work flexible traditional and non-traditional hours.
  • Ability to establish and maintain effective working relationships with Coalition partner agency staff, Board committees, volunteers, staff, donors and the general public.
  • Knowledge of Healthy Start, and other local health and human service providers.
  • Knowledge of Connect and local maternal-child health home-visiting programs
  • Knowledge of maternal-child health issues/community resources
  • Knowledge of Healthy Start Standards and Guidelines
  • Knowledge of motivational interviewing techniques
  • Professional communication, documentation and time management skills
  • Manage a caseload; organize and prioritize work assignments; make decisions utilizing independent judgment; determine the appropriate course of action in urgent or stressful situations
  • Ability to interface effectively with a diverse population
  • Must have strong and effective listening and verbal and written communication skills
  • Ability to accurately code and input data and information into a data system
  • Ability to initiate, coordinate and follow up on incoming and outgoing referrals
  • Computer skills with proficiency in Microsoft Office (Excel, Word, Outlook) and internet
  • Must be able to communicate effectively and be a team player at the Coalition.
  • Must be able to commit to the Coalition vision and be passionate about its mission.
  • Requires reliable transportation as travel is required, valid driver’s license, good driving record and automobile insurance.

Physical Requirements:

  • Ability to safely and successfully perform the essential job functions consistent with the ADA, and other federal, state and local standards.
  • Must be able to travel to alternate locations.
  • Required to sit, stand, walk, speak and hear while performing the duties of this job, regularly. The position requires extensive computer use so the employee must have sufficient hand dexterity to use a computer keyboard and be capable of reading a computer screen
  • Lifting/carrying up to 25 pounds.

Salary:

  • Salary: $71,500.00 per year

This position is dependent upon the continuation of funding. As such this position may terminate when the funding is no longer available.
