What You'll Do

• Provide direct patient care services to hospice patients and support the family after normal business hours
• Evaluate patient and family needs and assist in the execution of the patients plan of care
• Document and maintain clinical records
• Attend staff meetings and participate in Quality Improvement initiatives

JOB REQUIREMENTS & QUALIFICATIONS:

• Maintains an active nursing license that is in good standing in the state performing job duties.
• Maintains an active CPR certification.
• At least 2-5 years of experience in the healthcare management, hospice, or palliative care and/or training in end-of-life care, preferred.
• Proficient in MS Office applications and ability to learn department and job-specific software systems.
• Demonstrate organizational skills. Demonstrate effective verbal and written communication skills. Demonstrate analytical skills when problem-solving. Demonstrate high attention to detail and a high degree of accuracy.
