Care Program Manager

The Care Programs Manager is responsible for the implementation, management, operation, supervision, follow-up, networking, coordination, and evaluation of all the Sanitas USA Disease Management & Wellness programs. He/she works within the guidelines, policies and mission of the organization and will be accountable and responsible in reporting to the Executive leadership.

Job responsabilities:

• Create detailed implementation plans. This includes deadlines, milestones, processes, and risk mitigation protocols. Build a strong team through open communication and by collaborating on decision-making responsibilities.
• Collaborate with the regional Medical Directors on Disease Management Programs that serve the unique needs of their region. Share frequent feedback to the Care Programs team using the CASA framework.
• Define department job roles and responsibilities. Ensure that the Health Care Programs deliver a return in outcomes and contribute to efficient cost management.
• Identify new and creative program ideas to implement through continuous research and participation in regional and national education conference offerings.
• Participate in payer meetings as necessary. Support all LOBs and service line projects. Align Programs to meet the needs of internal and external providers, for example PHSO Affiliates Providers.

Education:

• BSN (Preferred)
• RN License (Preferred)

Other Knowledge, Skills and Abilities

• Excellent data analysis and reporting skills.
• Strong problem-solving, negotiation, and communication skills.
• Experience with industry-standard project management software and tools.
• Proficient software computer skills to include WORD, EXCEL, and other Microsoft Office programs.
• Experience with electronic medical records (EMR), eClinicalWorks is a plus.
• Knowledge of performance evaluation techniques and key metrics.
• Understanding of project management preferred.Must possess the skills to work in a physically and mentally demanding environment.
• Exceptional leadership, time management, facilitation, and organizational skills preferred.
• Must understand the healthcare continuum and have the ability to solve complex problems.
• Values consistent with culturally and family centered approach to care.
• Maintains patient confidentiality.
• Must be detail-oriented to ensure accuracy of reports and data. Ability to travel as required.
