We are seeking a full-time registered licensed nurse for our Hospice RN, Case Manager position. This candidate is responsible for the planning, coordination, and delivery of quality hospice care to individuals and their families in the home setting (which includes the community or agency setting).

Schedule: Monday – Friday 9:00AM – 5:30PM with on-call responsibility

Required Qualifications:

• Graduate of a state approved school of nursing.
• Certified (CHPN) preferred.
• One (1) year of nursing experience preferred.
• Prior home experience in hospice preferred.
• Current CPR (Basic Life Support).
• Must possess a valid driver’s license.
